Remove 4 duplicate gender doctrine entries flagged by QA:
- Steel Ministry: remove vanilla doctrine_gender_male_dominated
  (conflicts with custom doctrine_gender_male_preference)
- Terris: remove duplicate doctrine_gender_equal in custom block
- Survivorist: remove duplicate doctrine_gender_equal in custom block
- Cult of Ruin: remove vanilla doctrine_gender_male_dominated
  (conflicts with custom doctrine_gender_irrelevant)

Apply lore reviewer naming corrections:
- Rename tenet_metalmic_preservation → tenet_preservation_of_knowledge
  ("metalmic" is not a term from the books)
- Rename holy site "Kelsier's Pyre" → "The Survivor's Square"
  (Kelsier was killed in combat, not burned on a pyre)
