data(history): slim default provinces to skip vanilla-defined IDs

Regenerated 99_default_provinces.txt to only cover 2,413 province
IDs that have no history in either vanilla or our mod. Previously
covered all 13,164 non-mod IDs, causing ~30K duplicate-field
warnings from ck3-tiger for provinces vanilla already defines.
