feat(religion): set Lord Ruler as Steel Ministry religious head

Change steel_ministry_religion from doctrine_no_head to
doctrine_spiritual_head. Create d_steel_ministry_head religious
title ("Sliver of Infinity") and assign Lord Ruler (900100) as
holder. Add religious_head reference in steel_ministry faith block.
Also includes custom Scadrial doctrines, updated tenets, and
revised holy sites for all four faiths (3.8 changes).
