New `tooling/db/decision orphan-tickets` subcommand scans tickets with
a decision_ref that doesn't match any row in the decisions table.
Surfaces silently orphaned tickets from typo'd or renumbered D-IDs.
Makefile target: `make decisions-orphan-tickets`.
