remove duplicate pql gitignore block and hooks negation

The hooks under .pql/hooks/ are installed locally by pql init — they're runtime scaffolding, not source. The original block at lines 49-51 already covers .pql/* with the pql-plan.json negation; the second block was redundant. Co-Authored-By: Claude <noreply@anthropic.com>
